RHA Health Services, LLC is hiring an IDD Qualified Professional ICF to coordinate, monitor, and support person-centered services for individuals with IDD. You will lead PCP development, admissions, and OD activities alongside an interdisciplinary team to ensure seamless transitions into the facility.

The role emphasizes collaboration across nursing, direct support, clinical specialists, and guardians to uphold regulatory compliance and person-centered outcomes.

Qualifikationen

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field.
• At least 2 years of experience (in North Carolina) with individuals with IDD; 1 year required if Master’s degree is held.
• Experience developing, writing, and implementing Person-Centered Plans (PCPs).
• Working knowledge of ICF regulations, Medicaid/HCBS rules, and person-centered planning principles.
• Excellent communication, organizational, and documentation skills.

Aufgaben

• Lead the intake and admission process, including pre-admission screenings and visits with prospective individuals and their support teams.
• Coordinate all aspects of the admission process and ensure timely documentation and smooth transitions into the facility.
• Develop and implement the initial PCP upon admission with the IDT oversight and collaboration.
• Monitor occupancy by managing referrals and bed availability to maintain full occupancy.
