Job summary

OneWell Health Care seeks a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will assist with daily living activities, meal preparation, mobility, and personal care, while promoting independence and dignity.

Under supervision, you will implement ISP/IPP plans, monitor health and safety, and support community participation.

Qualifications

• High school diploma or equivalent.
• Prior caregiving or home health experience preferred.
• Experience in community-based or residential support settings is a plus.
• Ability to use electronic documentation and EVV systems.
• English proficiency required; bilingual helps depending on program needs.

Responsibilities

• Provide hands-on, person-centered support for individuals with IDD in home and community settings.
• Assist with daily living activities, meal prep, mobility, and personal care as authorized.
• Monitor health, safety, and emotional well-being; support transportation to appointments and community activities.
• Maintain accurate, timely documentation and participate in trainings and team meetings.
