OneWell Health Care in Pennsylvania seeks a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will help with daily living activities, health monitoring, and community participation, while following ISP/IPP and behavior support plans.

The role requires high school diploma or equivalent, strong documentation skills, and English proficiency.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support preferred.
• Ability to use electronic documentation and EVV systems.
• English proficiency required; bilinguals are a plus depending on program.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities and personal care, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain structured routines, support employment or vocational activities, and assist with transportation to appointments or community outings.
• Participate in trainings and team meetings; document accurately and timely.
