Job summary

OneWell Health Care in Pennsylvania is seeking a dedicated Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. The role focuses on daily living activities, health monitoring, and community inclusion while promoting independence and dignity.

The DSP will implement ISP/IPP plans under supervision, support transportation, documentation, and participate in trainings, with field-based work and flexible scheduling.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving or direct support preferred.
• English proficiency required; bilingual a plus.
• Ability to use electronic documentation and EVV systems.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community-based settings.
• Assist with daily living activities, meals, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain accurate and timely documentation and participate in trainings and team meetings.
