Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. The DSP helps with daily living activities, health monitoring, and community participation while promoting independence and dignity.

Qualifications

• High school diploma or equivalent is required.
• Experience in caregiving or home health is preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home/community settings.
• Assist with daily living activities, meal prep, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Document accurately in required records and participate in trainings/meetings.
