Job summary

OneWell Health Care seeks a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. The role focuses on daily living activities, skill development, health monitoring, and promoting independence and dignity.

The DSP works under supervision to implement ISP/IPP plans, with no supervisory duties. Requires English proficiency; field-based, flexible schedule, and a strong commitment to HIPAA and confidentiality are

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide consistent, hands-on, person-centered support in home and community settings.
• Assist with personal care, daily living activities, meal prep, mobility, and medication assistance as authorized.
• Maintain accurate and timely documentation and participate in trainings and team meetings.
• Support transportation to appointments or community activities and assist with scheduling flexibility.
