Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings across Pennsylvania. You will implement ISP/IPP plans, assist with daily living activities, and help promote independence and inclusion.

The role requires following care plans, maintaining documentation, and collaborating with an interdisciplinary team.

Qualifications

• Experience in caregiving, home health, or DSP preferred.
• Ability to follow written care plans and documentation guidelines.
• English proficiency required; bilingual is a plus depending on program.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal prep, mobility, and medication assistance as authorized.
• Monitor health, safety, and emotional well-being; promote independence and community participation.
• Document accurately via daily service notes, GPS verification, and incident reports.
• Participate in trainings, team meetings, and adapt to staffing needs.
