Job summary

OneWell Health Care seeks a dedicated Direct Support Professional to provide hands-on, person-centered support to individuals with IDD in home and community settings. This DSP role emphasizes daily living activities, skill development, health and safety monitoring, and promoting independence.

The DSP works under supervision to implement ISP/IPP and behavior support plans, maintaining documentation and participating in trainings.

Qualifications

• High school diploma or equivalent.
• English proficiency required; bilingualism a plus.
• Ability to use electronic documentation and EVV systems.
• Willingness to travel to individual homes and community sites.

Responsibilities

• Provide consistent, person-centered support per care plan.
• Assist with personal care and daily living activities; support with meals, mobility and medications as authorized.
• Implement behavior support strategies and monitor health, safety and wellbeing.
