Job summary

OneWell Health Care in Pennsylvania is seeking a dedicated Caregiver/Direct Support Professional (DSP) to support individuals with IDD in home and community settings. This field-based role emphasizes daily living activities, safety, and person-centered care with flexible scheduling.

You will implement ISP/IPP plans, assist with personal care, meals, mobility, transportation, and documentation, while following care plans and regulatory standards.

Qualifications

• High school diploma or equivalent required.
• Prior caregiving, home health, or DSP experience preferred.
• Experience in community-based or residential settings preferred.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with personal care, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and wellbeing.
• Maintain routines, support employment or vocational activities, and assist with transportation.
