Opportunity Village is seeking a Case Manager to provide compassionate case management, support, and advocacy for individuals with intellectual and/or developmental disabilities. The role involves liaison work with families, guardians, and service providers to ensure person-centered planning and meaningful community engagement.

Formación

• Bachelor's degree in a human services field is required.
• Must be qualified as a QIDP.
• CPI and CPR/First Aid certifications within 30 days of employment are required.
• Valid driver’s license with clean DMV record and reliable transportation.

Responsabilidades

• Provide case management, support, and advocacy for individuals with intellectual and/or developmental disabilities.
• Serve as liaison between individuals served, families/guardians, and service providers.
• Develop habilitation, service plans, and person-centered plans for individuals served.
• Train staff on PCP plans and maintain HIPAA-compliant records.
• Ensure compliance with safety regulations, OSHA, and agency policies.
• Coordinate transportation requests and document accurately.
