Job summary

Riverwood Center in Southwest Michigan seeks a dedicated behavioral health professional to provide community-based case management for individuals with intellectual and developmental disabilities and their families.

You will link, advocate, coordinate, and monitor services to help clients access health care, financial aid, employment, education, social services, and natural supports.

You will assess needs, develop and monitor Person-Centered Plans and document treatment.

Qualifications

• Bachelor in Social Work or Master’s Degree in an appropriate mental health discipline.
• QIDP, one year’s experience providing treatment to individuals with Intellectual and Developmental Disabilities.
• Must have reliable means of transportation for home visits and maintain Safe Driving Permit.

Responsibilities

• Provides case management services in the community to individuals with intellectual and developmental disabilities and their families.
• Facilitates linking, advocating, coordinating, and monitoring services to assist consumers in gaining access to health services, financial assistance, employment, education, social services, and natural supports.
• Assesses needs, develops, and monitors Person Centered Plans and documents treatment.
