Job summary

Service Coordination, Inc. seeks a Service Coordinator to provide community-based case management, advocating for individuals with disabilities and coordinating needed services.

Responsibilities include building rapport, developing Person-Centered Plans, coordinating resources, and ensuring compliance with HIPAA and state requirements. The role offers travel reimbursement, a quarterly bonus, and comprehensive benefits including tuition reimbursement and a 403(b) match.

Qualifications

• Bachelor's degree in a human services field or related field required.
• Experience with developmental disabilities preferred.
• Strong advocacy and education skills for individuals and families.

Responsibilities

• Provide case management services to individuals with developmental disabilities and/or intellectual disabilities.
• Establish positive working relationships with individuals, their families, and external stakeholders.
• Develop and monitor annual Person-Centered Plans (PCPs) and coordinate ongoing support services.
• Identify and secure ongoing support services and assist with transitions to community resources.
• Coordinate resources, refer to services, and document progress and outcomes.
• Adhere to HIPAA, confidentiality, and program policies; participate in trainings and meetings.
