Reliant Living Centers of Oklahoma, Inc. is seeking a compassionate Case Manager to support residents with intellectual and developmental disabilities in an ICF/IID setting.

You will coordinate IPPs, advocate for residents, and collaborate with families, guardians, and healthcare providers to ensure individualized plans and rights are respected. You will maintain accurate records, schedule appointments, and coordinate transportation while working with a multidisciplinary team to promote dignity,

Qualifications

• Associate degree in psychology or related field required.
• Bachelor's degree in psychology, social work, or related field preferred.
• Strong written and verbal communication skills.
• Ability to advocate for residents while respecting choices.

Responsabilités

• Coordinate IPPs and support plan development and review.
• Advocate for residents and help express concerns and choices.
• Coordinate services with families, guardians, and providers.
• Schedule medical appointments and arrange transportation.
• Attend planning meetings and document outcomes accurately.
• Maintain HIPAA-compliant records and follow facility procedures.
