Job summary

Agency for Persons with Disabilities seeks a Case Manager to assess and support residents, coordinate guardianship matters, and connect families with benefits. You will document progress, advocate for clients, and collaborate with the treatment team to ensure quality care.

Responsibilities include annual reports, court liaison, and coordinating services across centers, with a focus on compassionate, person-centered planning and compliance with state regulations.

Qualifications

• Bachelor's degree in a related human services field or equivalent.
• Experience with individuals with intellectual/developmental disabilities preferred.
• Strong counseling and advocacy skills; able to coordinate complex cases.

Responsibilities

• Prepare complete initial psychosocial assessment for new admissions.
• Develop and update psychosocial reports for each client on caseload.
• Coordinate interpretative conferences with parents/guardians about residents' programs.
• Monitor residents' finances and assist with benefits applications (SS, Medicaid).
• Advocate for residents and assist with guardianship, court proceedings, and consent issues.
• Document monthly visits and changes; ensure accurate life event records.
• Assist treatment team with goal development and client activities.
• Perform clerical duties and accompany residents to medical appointments as needed.
• Notify supervisors of significant events; perform other duties to support mission.
