Job summary

Benchmark Human Services is launching a Vermont program to support individuals with IDD and their families through planning, advocacy, and service coordination. Case Managers will help individuals live independently, participate in their communities, and access medical, social, and educational services.

We value diverse perspectives and require collaboration with guardians, families, and service providers to implement person-centered plans and ensure health and well-being.

Qualifications

• Must have VT residency and a valid driver’s license.
• Travel up to 75% of the time.
• Bachelor’s degree or equivalent with relevant human services experience required.

Responsibilities

• Complete Needs Assessment and Annual Review Protocol with the individual and team.
• Assist individuals in planning and scheduling person-centered planning meetings.
• Develop agendas and follow-up notes for planning meetings and share with the support team.
