Job summary

Benchmark Human Services in Vermont is seeking a dedicated Case Manager to support individuals with intellectual and developmental disabilities and their families, helping them access medical, social, educational, and other services to live independently in the community.

You will guide person-centered planning, perform needs assessments, and coordinate planning meetings with the individual, guardians, and support team, ensuring documentation accuracy and timely follow-up.

Qualifications

• Bachelor’s degree in any field plus 1 year of human services related experience.
• Associates degree in human services plus 2 years of related experience.
• High school diploma or equivalent plus 3 years of human services related experience.

Responsibilities

• Complete core competency trainings required by the State.
• Perform evaluations and assessments for individuals served.
• Support person-centered planning processes and assess needs.
• Schedule and facilitate participation in planning meetings.
• Develop and review the person-centered service plan with the individual and team.
• Coordinate documentation and ensure accessibility of information for planning.
