Benchmark Human Services seeks a qualified Case Manager in Vermont, Springfield area, to support individuals with intellectual and developmental disabilities. You will plan, advocate, monitor, and coordinate services with families, guardians, and support teams to help people live as independently as possible.

The role emphasizes person-centered planning, planning context documentation, and collaboration with guardians and service providers to ensure outcomes align with the individual’s goals.

Qualifications

• Must be able to work collaboratively while meeting individual performance goals.
• Verbal and non–verbal communication with a broad audience of stakeholders.
• Ability to use technology tools in daily performance of duties.

Responsabilités

• Complete Needs Assessment Annual Review Protocol with the individual and team.
• Assist the individual in discovery and planning discussions and identify goals and needs.
• Facilitate person-centered planning meetings and coordinate scheduling.
