Job summary

Benchmark Human Services is seeking a Case Manager to support individuals with intellectual and developmental disabilities in Barre, Vermont. You will plan, coordinate, and oversee medical, social, educational, and other services to help individuals live as independently as possible and participate in the community.

You will travel to Barre, Randolph, and Montpelier areas, collaborate with guardians and care teams, and ensure plans align with person-centered goals and health and welfare

Qualifications

• Candidates must understand Vermont's public service system for IDD and brain injury services.
• Knowledge of state statutes and program requirements is required.
• Ability to collaborate and communicate with diverse stakeholders.
• Proficiency using technology tools in daily duties.

Responsibilities

• Complete core competency training required by the State.
• Perform evaluations and assessments to meet individuals' needs.
• Support individuals in understanding goals in person-centered planning.
• Facilitate and schedule participation in person-centered planning meetings.
• Complete annual Needs Assessment reviews with individuals and teams.
• Assist in discovery, identifying preferences, goals, and needs for planning.
