Job summary

RHA Health Services, LLC is hiring a Qualified Professional to coordinate person-centered plans and oversee service delivery across residential and community settings. The role emphasizes strong communication, interviewing people served, and ensuring plans meet clinical and functional goals.

You will supervise Group Home Managers and Direct Care staff, review assessments, and monitor progress with the interdisciplinary team to support positive outcomes.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree preferred.
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities.
• Valid driver’s license, auto insurance, and reliable transportation.
• Must be able to lift between 20 and 50 pounds and meet CPR requirements.
• Knowledge of applicable regulations and ability to work with interdisciplinary teams.

Responsibilities

• Coordinate and monitor person-centered plans and progress toward personal, clinical and functional outcomes.
• Supervise and train Group Home Managers and Direct Care Associates.
• Conduct interviews to identify and communicate service needs for the people we serve.
• Complete incident reports and employee performance evaluations.
• Visit residential and vocational sites to ensure proper plan implementation.
• Support quality assurance and state survey readiness.
