Job summary

RHA Health Services, LLC is seeking a Qualified Professional to coordinate, monitor, and support person-centered services for individuals with IDD and mental health/substance use challenges. You will develop PCPs, manage admissions, and lead the referral process to maintain bed occupancy, collaborating with nursing, support staff, and guardians.

The role emphasizes independence, well-being and inclusion, requiring strong communication and documentation abilities and adherence to Medicaid/HCBS

Qualifications

• Develop, write, and implement Person-Centered Plans (PCPs).
• Working knowledge of ICF regulations and Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.

Responsibilities

• Admissions & Census Management: lead intake, pre-admission screenings, and PCP upon admission.
• Service Coordination & Planning: coordinate IDT meetings and translate needs into goals.
• Monitoring & Oversight: conduct site visits and assess PCP fidelity and outcomes.
• Advocacy & Communication: engage families/guardians and uphold residents' rights.
• Other Duties: ensure compliant documentation and participate in training and meetings.
