Job summary

RHA Health Services, LLC is hiring a Qualified Professional to coordinate, monitor, and support person-centered services for individuals with IDD and mental health needs in Burnsville, NC. The role centers on developing PCPs, admissions coordination, and ensuring full bed occupancy through effective referrals and intake management.

The QP will collaborate with nursing, DSPs, clinical specialists, administrators, and guardians to ensure regulatory compliance and positive outcomes.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field.
• 2 years of NC experience, 1 year in TN with IDD; 1 year required if Master’s.
• Demonstrated experience in developing, writing, and implementing Person-Centered Plans.
• Working knowledge of ICF regulations, Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Must be at least 18 years of age.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and meet-and-greet visits.
• Coordinate all aspects of admission and ensure smooth transitions into the facility.
• Develop and implement the initial PCP upon admission with the IDT.
• Maintain bed occupancy by active referral development and follow-up.
• Serve as primary IDT coordinator, schedule and document meetings on regulatory timelines.
• Interview individuals served to translate needs into actionable goals.
• Oversee development and implementation of PCPs based on assessments and input.
• Monitor delivery of services to ensure fidelity to PCP and outcomes.
