Job summary

RHA Health Services, LLC seeks an IDD Qualified Professional to coordinate person-centered services within an IDT in our ICF. You will lead admissions, develop PCPs, and drive occupancy through proactive intake and referrals.

You will collaborate with nursing, direct support staff, clinicians, and guardians to ensure outcomes align with goals and regulatory requirements. Strong communication and organizational skills are essential.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field (Master’s degree preferred).
• At least 2 years of NC experience, 1 year in TN with IDD; 1 year required if Master’s degree.
• Experience in developing, writing, and implementing Person-Centered Plans (PCPs).
• Working knowledge of ICF regulations, PCP principles, and Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Must be at least 18 years of age.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and PCP development upon admission.
• Coordinate all aspects of admission and ensure timely documentation and transitions into the facility.
• Serve as primary coordinator of IDT meetings and ensure PCPs are implemented and tracked.
• Monitor PCP fidelity and progress toward outcomes, and conduct regular site visits.
• Maintain communication with families and guardians; advocate for individuals served.
