Job summary

RHA Health Services, LLC is seeking a Qualified Professional (ICF) to coordinate services for individuals with intellectual and developmental disabilities and mental health/substance use needs. This PRN, hourly role requires strong communication and hands-on supervision of residential and day centers.

You will train staff, supervise managers and direct care staff, and ensure person-centered plans are implemented with quality and compliance.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus.
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities; 1-year experience if paired with Master’s degree.
• Working knowledge of applicable regulations that apply to our service offerings.
• Valid driver’s license, auto insurance, and reliable transportation.
• Ability to pass a drug screen and background check.
• Ability to lift between 20 and 50 pounds and meet the physical requirements of performing CPR.
• Supervisory/management experience and experience writing person-centered plans preferred

Responsibilities

• Center of the interdisciplinary service team to guide development of person-centered plans.
• Complete incident reports and employee performance evaluations.
• Identify and communicate person’s requests and needs for services.
• Supervise, integrate and coordinate person-centered plans and monitor outcomes.
• Review assessments and observations to help people meet full potential.
• Oversee residential activity and community integration to ensure safety and well-being.
• Recruit, hire, train and dismiss Group Home Managers.
• Serve as liaison between people served, families and guardians for service approval.
