Job summary

ECF Choices in Tennessee seeks a dedicated Support Coordinator to empower members with intellectual and developmental disabilities to live independently and gain employment. You will conduct intake, enrollments, and ongoing assessments, and develop Person-Centered Plans.

The role involves traveling to members’ homes in the Maury and Williamson counties, with mileage reimbursed, and an 8:00am – 5:00pm schedule with no on-call duties.

Qualifications

• RN with relevant certification (CDDN for RNs or DDC for LPNs) or related degree.
• BS in social work, nursing, education, or health care field; MSW/LC or equivalent preferred.
• QDDP/QIDP or 5+ years as a developmental disabilities professional or similar role.

Responsibilities

• Partner with members and families to identify supports to meet health, employment and independent living goals.
• Collaborate with clinical and social support teams to address member needs.
• Conduct face-to-face visits, assess needs, and document status across domains.
• Authorize home and community based services and ensure delivery aligns with needs.
• Maintain valid driver’s license and comply with vehicle policies.
• Complete required immunizations and digital literacy assessments.
