Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join the interdisciplinary care team. You will assess health needs, build care plans, coordinate care with providers, and educate members and families about conditions, treatments, and benefits.

Qualifications

• Active clinical licensure in nursing (RN/LPN) in the state where duties are performed.
• Bachelor's or Master’s in social work or related field is preferred.
• Certification in case management is preferred but not required.

Responsibilities

• Execute strategies and goals set by the leadership team to improve member experience.
• Contact plan members to assess health, develop care plans, and participate in care team meetings.
• Educate members and families about disease status, plan benefits, and resources.
