Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will perform comprehensive health assessments, develop and coordinate individualized plans of care, and educate members and families about disease status, benefits, and resources.

The role involves hybrid work with on-site field visits to facilities about 2–3 days a week and remote/home-office work for the remainder. RN/LPN licensure is required with strong case management skills.

Formación

• Active RN or LPN license in the state where duties are performed.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsabilidades

• Execute health assessments and develop care plans with the interdisciplinary team.
• Coordinate care and communicate plans with members, RP/POAs, and providers.
• Educate members and families on disease status, plan benefits, and resources.
• Collaborate with care team for appropriate treatments and follow-up.
• Document care team meetings and update care plans as needed.
