Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join our interdisciplinary team. You will assess, plan, implement, and coordinate care for seniors across communities, acting as a liaison among members, RP/POAs, and care teams.

Travel to facilities is expected, with some home-office time in a hybrid model. The role requires clinical practice experience in case management or related fields; an active RN/LPN license in the state is preferred, and a BS in social work is also acceptable.

Qualifications

• One year of clinical practice in case management, home health, or related area.

Responsibilities

• Execute strategies to improve member experience as directed by leadership.
• Contact plan members for comprehensive health assessment and care planning.
• Educate members and families about disease status, plan benefits, and resources.
• Collaborate with care team and medical directors for appropriate treatments.
• Follow up with members to monitor progress toward goals.
• Coordinate appointments and communication with RP/POAs and care team.
