Job summary

Curana Health, Inc. seeks a Care Ally, Case Manager to join our interdisciplinary care team. You will assess member health and social needs, plan and coordinate care, and liaise with RP/POAs, PCPs, and care partners to ensure timely access to services.

This hybrid role requires travel to assigned facilities 2–3 days per week and time working from a home office. RN or LPN licensure in the state, clinical case management experience, and comfort with Microsoft Office and multiple systems are

Qualifications

• 1 year of clinical practice in case management, home health, critical care, medical/surgical, discharge planning or concurrent review.
• Proficiency using Microsoft Office and ability to navigate multiple systems.
• Case management certification preferred; long-term care experience preferred.

Responsibilities

• Executes on strategies to manage and improve overall Member experience.
• Contacts Plan members to assess health, develop a plan of care, and participate in care team meetings.
• Educates members and families about disease status, plan benefits, resources and options.
• Collaborates with care team and medical directors to coordinate appropriate treatment.
• Follows up with members to monitor progress toward goals.
