Job summary

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team and support seniors across a network of communities. You will conduct assessments, develop care plans, educate members and families, and coordinate services in partnership with the care team and PCP.

The role requires an RN/LPN license or a BS in Social Work (MSW preferred), with travel to facilities 2–3 days per week and hybrid work from a home office.

Qualifications

• 1 year of clinical practice in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.
• RN or LPN license in state of performance OR BS in social work with MSW preferred.
• Proficiency in Microsoft Office (Word, Excel, Outlook) and navigating multiple systems.

Responsibilities

• Executes on strategies and goals to manage and improve overall Member experience.
• Contacts Plan members to conduct comprehensive health assessments and develop a plan of care.
• Educates member, family, and caregiver about disease status, plan benefits, and resources.
• Collaborates with care team and medical director to facilitate appropriate treatment.
• Follows up regularly with members to assess progress toward goals.
• Coordinates care team meetings, documents outcomes, and communicates updates to Plan.
