Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join our interdisciplinary care team. This role requires autonomy to manage field visits, coordinate with providers, and educate members and families about care plans and benefits.

The position supports hometown-based care with travel to assigned facilities, a hybrid work model, and collaboration with Plan members, RP/POAs, and care teams. Clinical experience in case management or home health is preferred.

Qualifications

• Ability to work autonomously in a hybrid role and manage field visits.
• Proficiency with Microsoft Office (Word, Excel, Outlook).
• Strong organization and ability to meet deadlines.

Responsibilities

• Execute plans and goals set by leadership to improve member experience.
• Conduct comprehensive health assessments and develop care plans with the ICT.
• Educate members and families as health coaches about disease status and plan benefits.
• Collaborate with care team and medical directors to coordinate treatments.
• Follow up with members to track progress toward goals and update plans.
