Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will assess, plan, implement, coordinate, monitor, and evaluate member health and social needs, acting as liaison between members, families, and care teams.

The role requires 1 year of clinical practice, ability to work autonomously in a hybrid setup, and willingness to travel 2–3 days per week to assigned facilities. A valid RN/LPN license or BSW/MSW degree is preferred.

Qualifications

• One (1) year of clinical practice experience in case management, home health, or related areas.
• Proficiency with Microsoft Office (Word, Excel, Outlook) and navigation of multiple systems.

Responsibilities

• Conduct health assessments, develop plans of care, and participate in interdisciplinary care team meetings.
• Educate members and caregivers about disease status, plan benefits, and community resources.
• Coordinate with care team and PCP to facilitate appropriate treatments and follow up with members.
