Job summary

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will assess member health and social needs, develop and implement a care plan, and coordinate with RP/POAs and care teams to optimize outcomes.

You will educate members and families, schedule appointments, and assist with home health coordination. Travel to assigned facilities 2–3 days weekly; remote work from home on other days.

Qualifications

• One year of clinical practice in case management or related areas.
• Proficiency using Microsoft Office and navigating multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Executes on strategies and goals set by leadership for managing and improving Member experience.
• Contacts Plan members to conduct a comprehensive health assessment and develop a plan of care.
• Serves as health coach to educate the member, family, and caregiver about disease status and plan benefits.
• Collaborate with the interdisciplinary care team and medical director to facilitate appropriate treatment.
• Routinely follows up with member to assess progress towards goals.
• Communicates health updates to RP/POAs and coordinates with care team for non-urgent questions.
