Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join an interdisciplinary team supporting seniors in community and facility settings. You will assess needs, plan care, coordinate services, and coach members and families on treatment options and benefits.

Active RN/LPN or a BSW/MSW is required, with case management certification preferred. This role involves travel to assigned facilities about 2–3 days per week, plus home-office work, and offers a hybrid work arrangement.

Qualifications

• One year of clinical practice experience in case management, home health, or related areas.
• Active RN or LPN license in the state where duties are performed, or a BSW/MSW degree.
• Case management certification preferred but not required.

Responsibilities

• Conduct comprehensive health assessments and develop care plans in collaboration with the interdisciplinary team.
• Coordinate care with members, RP/POAs, and care providers, including scheduling and follow-ups.
• Educate members and families about disease status, plan benefits, and available resources.
• Support contract development with preferred providers and assist with in-network coordination.
