Curana Health is hiring a Care Ally, Case Manager to join our interdisciplinary care team. This role coordinates health assessments, develops care plans, educates members and families, and collaborates with medical directors to ensure appropriate treatment and smooth care transitions.

The position requires clinical practice in case management and proficiency with Microsoft Office; hybrid work setup and travel to facilities are expected as part of the role.

Qualifications

• One (1) year of clinical practice experience in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Proficiency using basic computer skills in Microsoft Office (Word, Excel, Outlook) and ability to navigate multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.
• Case management certification preferred.

Responsabilités

• Executes on strategies and goals for managing and improving member experience.
• Contacts Plan members to conduct health assessments, develop care plans, and participate in care team meetings.
• Acts as health coach to educate members, families, and caregivers about disease status, benefits, and resources.
• Collaborates with care team and medical directors to facilitate appropriate treatment.
• Routinely follows up with members to assess progress toward goals.
• Communicates health updates to members and RP/POAs; supports care team questions.
• Supports scheduling of appointments and care coordination with providers.
• Coordinates with internal departments to resolve grievances, utilization management, and billing issues.
• Updates contact information for members and RP/POAs.
• Assists in identifying and securing contracts with preferred providers.
