Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts to provide in-home monitoring and complex care management for dually-eligible enrollees with medical, behavioral, and social needs. This role focuses on coordinating health services and community resources to improve outcomes and quality of life.

Qualifications

• ASN in nursing from an accredited program.
• RN license able to practice independently in MA.
• Prior care coordination/case management with dual-eligible populations preferred.
• Medicare/Medicaid managed care experience preferred.
• Clinical field/community-based training is a plus.

Responsibilities

• Engage enrollees in home and community settings to build complex care management relationships.
• Lead interdisciplinary care team (ICT) to develop holistic care plans.
• Coordinate with primary and specialty providers for seamless transitions.
• Assess enrollees with Comprehensive Assessments and functional screens.
• Educate members about benefits, services, and self-management.
