Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts. This RN-led role focuses on monitoring, follow-up, and clinical care management for dually-eligible enrollees with complex needs, coordinating with providers and community resources to improve health outcomes.

The position involves engaging with enrollees in home and community settings, leading the ICT, and ensuring continuity of care across services. Travel within MA is required.

Qualifications

• Active RN license in Massachusetts or eligible to practice
• AS in Nursing from an accredited program
• Experience in care coordination, case management or working with dual-eligible populations preferred

Responsibilities

• Lead interdisciplinary care team to develop holistic care plans
• Perform assessments and coordinate care transitions with providers and resources
• Educate members on benefits, chronic disease management and preventive care
• Ensure follow-up after hospitalizations to prevent readmissions
