Job summary

Commonwealth Care Alliance is seeking an Integrated Care Clinical Manager (RN) in Massachusetts. This community-based role provides monitoring, follow-up, and care management for dually-eligible enrollees with complex medical, behavioral, and social needs, coordinating services to improve outcomes.

Qualifications

• ASN degree in nursing required.
• RN license required.
• Prior care coordination or case management experience preferred.
• Medicare/Medicaid managed care experience preferred.
• Community-based care experience a plus.

Responsibilities

• Engage with enrollees in homes and community settings to build complex care management relationships.
• Liaise between healthcare providers, resources, and enrollees for seamless care transitions.
• Conduct comprehensive assessments and functional assessments as appropriate.
• Lead the interdisciplinary care team to develop holistic care plans.
• Oversee enrollee utilization of long-term services and supports to support staying in their preferred location.
• Educate members and families about benefits and preventive care.
• Coordinate with providers and community organizations to avoid service duplication.
• Follow up after hospitalizations to ensure continuity of care.
