CareSource is seeking an Integrated Care Clinical Manager (RN) in Massachusetts to coordinate care for dually eligible enrollees with complex needs. The role emphasizes home-based monitoring, follow-up, and coordination of medical, behavioral, and social services.

You will lead an interdisciplinary team, oversee care plans, and ensure seamless transitions across providers and community resources while promoting prevention and self-management. Travel to member homes is required.

Formación

• Associate of Science in Nursing required.
• Registered Nurse with ability to independently serve people with complex needs.
• Prior care coordination or dual-eligible experience preferred.

Responsabilidades

• Engage enrollees in home and community settings to build a complex care management relationship.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate long-term services to keep enrollees in their preferred location.
• Educate members about benefits and manage chronic conditions.
