Job summary

Qualifications

• Strong interpersonal and communication skills with members, families, and providers.
• Ability to manage multiple cases and maintain attention to detail.

Responsibilities

• Engage enrollees in homes and community settings to build complex care management relationships.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate with providers, community resources, and state agencies to avoid service duplication.
• Oversee enrollee utilization of long-term services and supports for desired living locations.
• Educate enrollees about benefits, self-management, and preventive care.
