CareSource seeks a Hybrid RN Care Manager to coordinate comprehensive care for dually-eligible enrollees with complex medical and social needs in the North Shore and Merrimack Valley MA region. The role involves home visits, care planning, and collaboration with healthcare providers to improve outcomes and quality of life.

Responsibilities include conducting assessments, leading the ICT, and ensuring smooth transitions across services while adhering to NCQA standards.

Formación

• AS in nursing from an accredited program.
• Licensed RN in Massachusetts.
• Experience in care coordination or dual-eligible populations preferred.

Responsabilidades

• Engage with enrollees in homes and community settings to build complex care management relationships.
• Coordinate with ICT to create holistic care plans addressing medical and non-medical needs.
• Lead interdisciplinary team and ensure care transitions between providers.
• Oversee enrollee utilization of long-term services and supports to keep members in their chosen location.
• Assist members in accessing housing, transportation, and other social services.
