CareSource is seeking an Integrated Care Clinical Manager in Massachusetts to coordinate care for dual-eligible enrollees with complex medical, behavioral, and social needs. Qualifikationen

• AS in nursing from an accredited program required.
• Licensed RN in Massachusetts with ability to serve dual-eligible populations.
• Experience in care coordination or case management preferred.

• Engage with enrollees in homes and community settings to build complex care management relationships.
• Coordinate with healthcare providers and community resources to ensure seamless care transitions.
• Perform comprehensive assessments including functional assessments and risk evaluations.
• Lead the interdisciplinary care team to develop holistic care plans addressing medical and social needs.
• Travel within Massachusetts to support members and coordinate services.
