Commonwealth Care Alliance Inc. is seeking a Hybrid RN Case Manager for South Shore and Southeast Massachusetts to coordinate care for dually eligible enrollees with complex needs. You will monitor, follow up, and implement care plans in collaboration with an interdisciplinary team.

The role requires in-home and community-based visits, education on benefits, and navigation of housing, transportation, and social services. Travel is required within the territory.

Formación

• AS nursing degree from an accredited program.
• Current RN license in Massachusetts.
• Experience in care coordination or dual-eligible populations preferred.
• Medicaid/Medicare knowledge and managed care exposure preferred.

Responsabilidades

• Engage with enrollees in homes and community settings for complex care management.
• Coordinate with providers and community resources for seamless care transitions.
• Perform comprehensive assessments and develop care plans with ICT.
• Educate members and families about benefits and chronic disease management.
• Travel to member and provider sites as required and ensure NCQA adherence.
