Job summary

CareSource in Massachusetts is seeking a Hybrid RN Care Manager to coordinate care for dually eligible enrollees with complex needs in Western MA. You will perform comprehensive assessments, develop care plans, and collaborate with ICT to ensure seamless transitions and access to community resources.

Travel across Springfield, Longmeadow, Northampton, Sunderland, Chicopee is expected; RN license in MA required.

Qualifications

• ASN degree from an accredited nursing program.
• RN license in Massachusetts with ability to independently care for complex cases.
• Case management or care coordination experience preferred.
• Medicare/Medicaid knowledge a plus.

Responsibilities

• Engage with enrollees in home and community settings to develop care plans.
• Lead interdisciplinary care team to create holistic plans addressing medical and social needs.
• Coordinate access to community resources, housing, transportation and food aid.
• Educate members and families on benefits, chronic disease management, and preventive care.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
• Travel to member, provider, and community sites as part of the role.
