Job summary

Commonwealth Care Alliance Inc. is seeking a Hybrid RN Care Manager for the North Shore and Merrimack Valley region in Massachusetts. The RN will monitor, follow up, and manage care for dually-eligible enrollees with complex needs in home and community settings.

This role emphasizes coordinating health services with social supports to improve outcomes and quality of life. The position requires an RN license in MA, experience in care coordination, and strong communication skills.

Qualifications

• ASN in Nursing from an accredited nursing program is required.
• Current RN license in Massachusetts is required to practice.
• Care coordination or case management experience preferred.
• Medicare/Medicaid managed care knowledge preferred.

Responsibilities

• Engage enrollees in their homes and community settings to establish a complex care management relationship.
• Lead the interdisciplinary care team to create holistic care plans.
• Coordinate with providers and community resources to ensure seamless care transitions.
• Travel to member, provider and community visits as required to support enrollment and care.
• Educate members about benefits, preventive care, and available services.
