Job summary

CareSource in the Greater Boston area is seeking an Integrated Care Clinical Manager (RN) to provide in-home and community-based care management for dually-eligible enrollees with complex medical and social needs.

Responsibilities include leading the ICT, coordinating with healthcare providers, and ensuring continuity of care across Medicare and Medicaid services. Requires MA RN license, and travel to multiple Boston-area sites.

Qualifications

• Associates of Science in nursing from an accredited program.
• Licensed Registered Nurse with ability to serve people with complex needs.
• Experience in care coordination or dual-eligible populations preferred.

Responsibilities

• Engage enrollees in homes and community settings to build care plans.
• Coordinate between providers, resources, and enrollees for seamless care.
• Lead interdisciplinary care team to address medical and social needs.
• Monitor enrollee utilization of long-term services and supports.
• Educate members about benefits, chronic disease management and preventive care.
• Ensure follow-up after hospitalizations to prevent readmissions.
• Document care plans and communicate updates to stakeholders.
