Commonwealth Care Alliance is seeking a dedicated Registered Nurse for a Hybrid RN Case Manager role serving the South Shore and Southeast MA. You will monitor, coordinate, and manage care for dually eligible enrollees with complex medical, behavioral, and social needs in home and community settings.

You will lead the interdisciplinary team, collaborate with providers, and ensure seamless transitions across services while promoting quality outcomes and patient-centered care.

Competenze

• Associates of Science degree in nursing required.
• RN license required and able to independently serve people with complex needs.
• Experience in care coordination or case management with dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical field/community-based training a plus.

Mansioni

• Engage enrollees in homes and community settings to build a complex care management relationship.
• Coordinate with providers and community resources for seamless care transitions.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non‑medical needs.
• Monitor enrollee utilization of long-term services to keep them in their preferred location.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
• Travel to member, provider, and community visits as required.
