Job summary

CareSource seeks a Hybrid RN Care Manager for the North Shore/Merrimack Valley region. You will monitor, follow up, and coordinate clinical care for dually-eligible enrollees with complex needs, integrating health services with community resources to improve outcomes.

The role emphasizes in-home engagement, collaboration with an interdisciplinary team, and education on benefits and preventive care. Requires MA RN licensure and travel across MA.

Qualifications

• Active RN license in Massachusetts.
• Experience in care coordination or case management.
• Medicare/Medicaid managed care experience preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build strong care management relationships.
• Collaborate with care team to develop holistic care plans addressing medical and social needs.
• Perform assessments and coordinate with providers to ensure timely care transitions.
• Travel to member homes and community locations as required.
• Educate members about benefits, services, and preventive care.
