Job summary

CareSource is seeking a Hybrid RN Care Manager for the North Shore and Merrimack Valley in Massachusetts. The role focuses on complex care management for dual-eligible enrollees, coordinating with providers and community resources to improve health outcomes.

The position requires an RN with strong care coordination skills, a valid MA license, and willingness to travel within the territory. A $5K sign-on bonus is offered and a full-time schedule is provided.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited nursing program required.
• A Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs. required.
• Prior experience in care coordination, case management, or working with dual-eligible populations preferred.
• Medicaid and/or Medicare managed care experience preferred.
• Clinical Field/ Community Based Training a Plus.

Responsibilities

• Engage enrollees in their homes and community settings to build complex care management relationships.
• Coordinate with ICT to create holistic care plans addressing medical and non-medical needs.
• Lead inter disciplinary team and collaborate with providers to ensure smooth transitions of care.
• Oversee enrollee utilization of long-term services and supports to keep them in their preferred location.
• Follow up after hospitalizations or significant events to ensure continuity of care.
