Job summary

CareSource in Massachusetts is hiring a Hybrid RN Care Manager to provide care coordination for dually eligible enrollees with complex medical, behavioral, and social needs in Metro West and Central MA.

You will lead the interdisciplinary care team, develop holistic care plans, engage members in homes or community settings, coordinate with providers and community resources, and educate families about benefits and chronic condition management.

Qualifications

• Associates of Science degree in nursing from an accredited program.
• Registered Nurse licensed in Massachusetts.
• Experience in care coordination or case management with dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical field/community-based training a plus.

Responsibilities

• Engage enrollees in their homes and community settings to establish a complex care management relationship.
• Lead the interdisciplinary care team and develop holistic care plans addressing medical and non-medical needs.
• Coordinate with healthcare providers and community resources to ensure seamless care transitions.
• Oversee enrollee utilization of long-term supports and ensure access to services.
• Educate members and families about benefits, chronic condition management, and preventive care.
