Mass General Brigham Community Physicians is seeking a patient navigator to work closely with patients, families, and healthcare providers to ensure a seamless, patient-centered experience by coordinating resources and guiding through the healthcare system. You will advocate for patients, connect them with community services, educate on conditions and treatments, and maintain thorough documentation.

The role is full-time, hybrid (1 day remote, 4 days in offices at Newburyport and Haverhill), 40

• Bachelor's degree in a related field required; relevant experience may substitute.
• 1+ years of patient-facing or community health experience strongly preferred.

Responsabilidades

• Advocate for patients and families, ensuring needs are considered in care planning.
• Serve as liaison between patients, providers, and stakeholders.
• Coordinate patient care services, appointments, and follow-up plans.
• Assist patients in understanding and adhering to care plans.
• Connect patients with healthcare and community resources, transportation, and financial assistance.
• Educate patients and families about conditions, treatments, and self-care.
• Empower patients to participate in healthcare decisions.
• Maintain accurate documentation of interactions, care plans, and referrals.
